If you are suffering from a chronic health condition, ask your GP for an Enhanced Primary Care (EPC). You could be eligible for rebates for up to 5 sessions per year. Gap payments range from $15-$30 per consult. Please see the forms below for further info.

Should you not be eligible for an EPC referral, you can still see one of our dietitians. A private fee will be charged, however may be rebated by your health fund. An initial consult is charged at $100.00, and review consults $70.00 per session.

If you have a DVA card, you could be eligible for regular sessions with a dietitian under a D904 referral. Consults will be bulk-billed and home visits may be possible. Please get in contact for further details or to check your eligibility.
